The Community Care Social Worker forms part of Peninsula Health's Independence Program and is based on the Wagner Chronic Care Model, and evidenced based systems framework.
The Community Care Social Worker is responsible for coordinating intensive short term (3-6 months) outreach care and delivering allied health interventions within their scope of practice to clients who have chronic health conditions and / or complex healthcare needs.
The role involves facilitating collaborative integrated care planning across acute and community based services. Care coordinators support people to navigate the service system and services. They provide the point of contact as people undertake their care plan.
- Provide safe, cost effective outreach assessment and multi-disciplinary care planning within a social model of health considering its impact on positive health outcomes.
- Understand Chronic Disease processes and their impact on the client/carer.
- Understand the concept of health literacy, the individual level of health literacy and the impact this has on a client's ability to manage their care. .
- Provide allied health services including thorough assessment, intervention and discharge planning within scope of practice.
- Demonstrate current evidence based practice within your area of clinical expertise.
- Ability to work with clients on transition to other services once goals are complete.
- Qualification in Social Work with the eligibility of the AASW.
- Current Victorian Driver's License.
- Demonstrated experience working with at-risk and vulnerable people with complex needs.
- Demonstrated knowledge and experience regarding chronic disease management.
- Ability to manage multiple competing tasks and effectively prioritise.
- Demonstrated Social Work experience in comprehensive assessment and interventions in a home setting.

Selection Criteria Essential: Qualified Social Worker with eligibility of the AASW.
Essential: Demonstrated Social Work experience in providing comprehensive Social Work assessment and interventions in a home setting.
Essential: Demonstrated knowledge and experience regarding chronic disease management.
Essential: Demonstrated experience working with at-risk and vulnerable people with complex needs.
Essential: Advanced demonstrated knowledge and application of Social Work theory (e.g. crisis intervention, strengths based).
Essential: Demonstrated experience in working both independently and as a part of the multidisciplinary team, with the ability to work effectively, supportively and flexibly.
Essential: Demonstrated commitment to professional development and evidence based practice.
